不同剂型硝苯吡啶对高血压病患者心率变异性的影响

Influence of Different Formulations Of Nifedipine On Heart Rate Variability, In Hypertensive Patients

  • 摘要: 目的 在比较常规剂型 (CF)与控释剂型 (GITS)硝苯吡啶 (N)降压效果的基础上 ,以心率变异性 (HRV)为观察指标 ,评价两者对高血压病患者自主神经功能的影响。方法 高血压病患者 2 5例 ,采取自身对照方式 ,先后服用N CF与N GITS各四周 ,监测血压波动 ;于第四周、第八周末分别行Holter检查及HRV分析。观察降压效果 ,在两种药物间比较 2 4小时HRV变化 ,同时比较服用某种药物后不同时段的 5分钟HRV变化 (第Ⅰ时段 5 :30AM~ 5 :35AM ,第Ⅱ时段 10 :0 0AM~ 10 :0 5AM )。结果 N GITS使SBP、DBP下降的绝对值 (32 .0 0± 2 3.97,19.2 5± 10 5 5 ,mmHg)明显大于N CF(2 4 .5 0± 2 4 .5 4 ,15 .0 0± 9.73,mmHg ,P <0 0 0 1) ;服用N GITS后的 2 4小时RMSSD与PNN50 (2 5 .70± 9.5 1,5 .33± 5 .0 7)明显大于服用N CF后 (2 2 .2 2± 10 5 2 ,3.4 0± 3.4 1,P <0 0 5 ) ,5分钟HRV中第Ⅱ时段的TP、LFn与LF/HF均大于第Ⅰ时段 (814 .5 7± 5 70 71vs371.6 3± 336 .5 0 ,6 6 .84± 2 4 .72vs5 0 16± 2 3.83,4 .6 6± 4 .88vs 2 .0 1± 3.11,P <0 0 5 ) ,且HRV仍有昼夜节律性 ;服用N CF后第Ⅰ时段的LFn大于第Ⅱ时段 (6 7.88±14 .6 5vs5 4 .12± 2 1.85 ,P <0 0 5 ) ,第Ⅱ时段的TP大于第Ⅰ时段 (15 14 .30

     

    Abstract: Aim To compare the effects of the conventional fast release(N CF) and nifedipine gastro intestinal therapeutic system (N GITS) on blood pressure and heart rate variability(HRV) in hypertensive patients. Methods Twenty five essential hypertensive patients were given N CF in the first 4 weeks and N GITS in the following 4 weeks. Holter monitor was carried out at the end of 4th and 8th week. Both 24 h and 5 min (Time Ⅰ 5:30AM~5:35 AM, Time Ⅱ 10:00 AM ~10:05 AM) HRV were analyzed. Results N GITS reduced blood pressure more significantly (32.0±23.9, 19.3±10 6 mmHg) than N CF (24.5±24.5, 15.0±9.7 mmHg, P <0 001). After taking N GITS, the normal difference of day time and night time HRV was reserved; the rMSSD and PNN\-\50\ in 24 hour HRV (25.7±9.5, 5.3±5.1) were much higher than those after N CF(22.2±10 5, 3.40±3.41, P <0 05); and the TP, LFn and LF/HF in 5 minute HRV were also much higher in Time Ⅱ(814.6±570 7, 66.8±24.7, 4.7±4.9) than in Time Ⅰ(371.6±336.5, 50 2±23.8, 2.0±3.1, P <0 05). After taking N CF,in 5 min HRV, the TP in Time Ⅱ(1514.3±1495.1) was higher than in Time Ⅰ(472.9±887.9, P <0 05); but the LFn in Time Ⅰ(67.9±14.7) was higher than in Time Ⅱ(54.1±21.9, P <0 05); and there was no difference of LF/HF between these two times. Conclusion The N GITS had better antihypertensive effect than the N CF. The N GITS did not activate the sympathetic nerve system but increase the vagus nerve activity with the reservation of the circadian rhythm of HRV.
