氨氯地平对糖尿病大鼠肾脏的保护作用

The Renal Protective Effect of Amlodipine in Diabetic Rats

  • 摘要: 目的探讨苯磺酸氨氯地平对糖尿病大鼠肾脏细胞凋亡的影响。方法 21只 SD 雄性大鼠分为3组:正常对照组(对照组,n=6)喂以普通饲料;余15只大鼠高糖高脂膳食喂养6周后,一次性腹腔内注射小剂量链脲佐菌素(streptozotocin,STZ)30 mg/kg;2周后血糖升高≥16 mmol/L 者成模,共12只,再随机分为糖尿病组(糖尿病组,n=6)和糖尿病大鼠应用苯磺酸氨氯地平治疗组(治疗组,1 mg/kg·d,n=6)。12周后光镜观察各组。肾脏病理改变;流式细胞术检测大鼠。肾组织细胞凋亡率;检测各组内生肌酐清除率(Ccr)和24 h 尿白蛋白的排泄率(Uaer)。结果 12周后,糖尿病组大鼠肾小球细胞外基质增生,部分。肾小管轻度萎缩或管腔扩张,上皮细胞肿胀,胞浆内可见脂肪空泡变性,但治疗组较糖尿病组病变减轻;糖尿病大鼠较正常组的肾细胞凋亡率明显升高,氨氯地平能明显降低肾细胞凋亡率对照组(13.8±1.3)% vs 糖尿病组(26.5±2.7)%vs 治疗组(20.0±2.0)%,P<0.05,增加肌酐清除率对照组(0.110±0.020)vs 糖尿病组(0.021±0.001)vs 治疗组(0.065±0.004)mL/min,P<0.05,降低蛋白排泄率对照组(0.081±0.001)vs 糖尿病组(0.150±0.004)vs 治疗组(0.07±0.01)mg/24 h,P<0.05。结论苯磺酸氨氯地平可减轻糖尿病大鼠肾脏病理损害,减少糖尿病大鼠肾脏细胞凋亡,可使 Ccr 下降减轻、Uaer 排泄减少,以达肾脏的保护作用。

     

    Abstract: Objective To study the effects of amlodipine on renal damage in diabetic rats.Methods Diabetic rat model was established by feeding high-glucose,high-lipid for 6 weeks,and intraperitioneal injection of single dose streptozotocin (STZ;30 mg/kg).SD rats were randomized to normal control (control group,n=6),diabetic without treatment(n=6) or with amlodipine treatment(1 mg/kg·d,n=6)for 12 weeks.The renal pathological changes were determined by microscopy;and cellular apoptosis by flow cytometry.The creatinine clearance (Ccr) and urine albumine excretion (Uaer) were determined before and after treatment.Results Amlodpine attenuated the increased extracellular matrix in glomerulus,renal tubules morphological disorder and swollen epithelial cells or vacuolar degeneration in diabetes rats.Amlodipine reduced apoptosis rate of renal cells (apoptosis rate:control group 13.8%±1.3% vs diabetes group 26.5%±2.7% vs amlodipine group 20.0%±2.0%,P<0.05),increased Ccr:(control group 0.110±0.020 vs diabetes group 0.021±0.001 vs amlodipine group 0.065±0.004 mL/min,P <0.05),and decreased Uaer:(control group 0.081±0.001 vs diabetes group 0.150±0.004 vs amlodipine group 0.07±0.01 mg/24 h,P<0.05).Conclusion Amlodipine protects kidney manifested by reduction of pathologic renal tubule disorder,decreases in renal apoptosis,increases in Ccr,and decreases in Uaer in diabetic rats.
