腹膜透析患者持续容量超负荷使大动脉僵硬度增高
Sustained Volume Overload was Associated with Increases in Stiffness of Great Artery in Continuous Ambulatory Peritoned Dialysis Patients

摘要: 目的探讨持续非卧床腹膜透析(CAPD)患者中持续容量超负荷与大动脉僵硬度之间的关系。方法采用纵向队列研究,选取CAPD患者220例(男性86例,女性134例)为研究对象,每月评估1次容量状态,连续观察6月。根据容量负荷水平分为3组,容量正常组(正常组,n=94)、间断容量超负荷组(间断组,n=79)、持续容量超负荷组(持续组,n=47)。多频生物电阻抗分析仪测定患者的容量状态。大动脉僵硬度通过颈股动脉脉搏波传导速度(cf-PWV)评估。常规标准测量相关生化指标。分析6月前后大动脉僵硬度的差异。结果基础资料中,3组的收缩压有差异持续组(144.8±20.9)比间断组(135.5±23.4)、正常组(127.0±23.9)mmHg,P<0.01或P<0.05,6月后,3组的收缩压仍保持不同持续组(145.4±22.0)比间断组(136.8±24.5)、正常组(130.0±26.0)mmHg,P<0.01或P<0.05。脉搏波传导速度(PWV)数值在持续组(12.6±1.6)m/s和间断组(12.2±2.3)m/s均明显大于正常组(10.7±2.1)m/s,均P<0.01。血白蛋白在正常组(39.6±6.5)g/L明显高于间断组(38.6±4.0)g/L和持续组(36.9±4.0)g/L,均P<0.05。正常组和间断组6月前后PWV均无明显变化,只在持续组明显增高,并有收缩压增高,血白蛋白减少,高敏C反应蛋白增加。结论持续容量超负荷伴有大动脉僵硬度增大,可能与收缩压增加,血白蛋白减少,高敏C反应蛋白增加有关。Abstract: Objective To investigate the relationship between continuous volume overload and large arterial stiffness in patients receiving continuous ambulatory peritoned dialysis(CAPD).Methods All clinically stable CAPD patients(n=220) at our centre were followed up monthly using multi-frequency bioelectrical impedance analysis to assess volume status and carotid-femoral pulse wave velocity(PWV) as an index of large arterial stiffness for 6 months.Results All patients completed the study Male/Female(86∶134),94 were classified as normal volume(NV),79 intermittent volume overload(IVO) and 47 sustained volume overload(SVO).At baseline the systolic blood pressure(SBP) was significantly higher in sustained volume overload SVO(144.8±20.9) vs IVO(135.5±23.4) vs NV(127.0±23.9) mmHg,P<0.05 or P<0.01;after 6 months this difference remained significant SVO(145.4±22.0) vs IVO(136.8±24.5) vs NV(130.0±26.0)mmHg,P<0.05 or P<0.01.PWV in both the SVO and IVO were significant higher than that in NV(12.6±1.6),(12.2±2.3) vs(10.7±2.1) m/s,respectively,both P<0.01.The serum albumin in the SVO and IVO were both significantly lower than the NV patients (36.9±4.0) vs(38.6±4.0) and(39.6±6.5) g/L,respectively,both P<0.05.No significant changes were observed in PWV for the NV or IVO patients,however in the SVO patients PWV was increased associated with lower albumin,elevated SBP and hsCRP.Conclusion Our date study demonstrates that continuous volume overload is strongly associated with increased large arterial stiffness in CAPD patients,which might be related to blood pressure,or nutritional status.
