不同降压药物对原发性高血压伴左心室肥厚患者血浆脑钠尿肽及左心室质量指数的影响

Effects of different antihypertensive medications on plasma brain natriuretic peptide and left ventricular mass index in hypertensives with left ventricular hypertrophy

  • 摘要: 目的观察不同降压药物对原发性高血压(EH)伴左心室肥厚(LVH)患者血浆脑钠尿肽水平及左心室质量指数(LVMI)的影响。方法入选某3级甲等综合医院心内科门诊及住院EH伴LVH患者224例,随机分为4组,分别给予氢氯噻嗪+贝那普利、氢氯噻嗪+坎地沙坦、氢氯噻嗪+硝苯地平、氢氯噻嗪+美托洛尔治疗12周。于服药前及12周末分别测定各组血浆脑钠尿肽浓度、左心室质量指数(LVMI)及舒张早期二尖瓣口峰值血流速度(E峰)与舒张晚期峰值血流速度(A峰)比值(E/A),并对脑钠尿肽和LVMI、E/A进行相关性分析。结果药物治疗组各组与治疗前相比,血浆脑钠尿肽浓度、LVMI降低,E/A升高。组间比较,LVMI差异无统计学意义,氢氯噻嗪+坎地沙坦组脑钠尿肽及E/A的改善程度优于其他治疗组(P<0.05)。药物治疗组血浆脑钠尿肽水平与LVMI呈正相关(治疗前r=0.47,治疗后r=0.45,P<0.01);血浆脑钠尿肽水平与E/A呈负相关(治疗前r=-0.41,治疗后r=-0.52,P<0.05)。结论降压治疗早期,血管紧张素受体拮抗剂在降低血浆脑钠尿肽水平及改善左心室舒张功能上可能具有一定的优势。

     

    Abstract: Objective To investigate the effects of different antihypertensive agents on plasma brain natriuretic peptide(BNP)and left ventricular mass index(LVMI)in hypertensive patients with left ventricular hypertrophy(LVH). Methods A total of 224essential hypertensive inpatients and outpatients with LVH from a tertiary A hospital were enrolled and divided into four groups. These subjects were treated by the combination of thiazad with benapril,candesartan,nifedipine CR,or with metroprolol respectively for twelve weeks. Plasma BNP,LVMI and the ratio of mitral flow early diastole(E)to atrial contraction(A)(E/A)were monitored before and after the therapy. The correlation between BNP and LVMI,as well as E/A was analyzed. Results The BNP levels and LVMI were reduced while E/A increased significantly after treatment than those before treatment. BNP and E/A were improved more obviously in combined regimen of thiazad plus candesartan than the other therapeutic schemes,although no statistical significant difference in LVMI was found between the treated groups. BNP levels were positively correlated with LVMI before(r=0.47)and after(r=0.45)the treatment(all P<0.01),whereas negatively related to E/A at the initiation(r=-0.41)and the end(r=-0.52)of therapeutic course(all P<0.05).Conclusion At the early stage of antihypertensive remedy,angiotensin receptor blocker may have advantages in decreasing plasma BNP levels and ameliorating left ventricular diastolic function.
