Abstract:
Objective To investigate the relation of epicardial adipose tissue volume(EATV),determined by dualsource CT(DSCT)cardiac imaging,to levels and circadian rhythms of blood pressure. Methods The data of 301 subjects receiving DSCT coronary angiography were collected,while EATV of each subject was measured by DSCT. Of the 301subjects,91with hypertension received ambulatory blood pressure monitoring(ABPM)during their stay at the hospital. Meanwhile,91patients with hypertension were divided into four groups according to quartiles of EATV,and different 24-hour ABPM parameters of the four groups were observed. Multivariate logistic regression analysis was used to study the correlation between EATV and hypertension. Results EATV of the hypertension group was significantly higher than that of the non-hypertension group(103.85±41.32)vs(92.35±41.59)cm3,P <0.05. After excluding obesity,diabetes and other cardiovascular risk factors,EATV still was an independent risk factor for hypertension(P<0.05);The EATVs of the non-dipper hypertension group and reverse dipper hypertension group were significantly higher than that of the dipper hypertension group among patients receiving dynamic electrocardiogram examination(108.26±31.06),(119.83±28.45)vs(99.26±29.45)cm3,both P<0.05. Mean 24-hour systolic blood pressure(24hSBP),mean 24-hour diastolic blood pressure(24hDBP),mean daytime SBP(dSBP),mean daytime DBP(dDBP),mean nighttime SBP(nSBP),mean nighttime DBP(nDBP)and other parameters of the four groups,divided in accordance with quartiles of EATV,progressively increased with the increase of EATV(all P<0.01). Conclusion Increasing EATV is associated with the occurrence of hypertension,and it can aggravate abnormal circadian rhythm of blood pressure.
