高血压合并肥胖患者血清网膜素1水平及其与动脉弹性的相关性
The relationship between serum omentin-1 levels and arterial elasticity among essential hypertensive patients with obesity

摘要: 目的探讨原发性高血压与肥胖患者血清网膜素1水平的变化,及网膜素1与动脉弹性的相关性。方法收集正常血压和原发性高血压患者共255例,年龄(56.7±7.8)岁。根据血压及体质量指数(BMI)分为:正常对照组65例,单纯高血压组(H组)66例,单纯肥胖组(O组)63例,高血压合并肥胖组(HAO组)61例。采用双抗夹心酶联免疫吸附试验法测定网膜素1,动脉硬化诊断仪测量臂踝动脉脉搏波传导速度(baPWV)。结果与正常对照组比较,H组、O组及HAO组网膜素1水平下降分别为(176.7±53.4)、(151.4±48.2)、(143.3±47.2)比(233.7±57.5)μg/L,均P<0.01;HAO组网膜素1水平较H组下降(P<0.01),较O组有下降趋势,但差异无统计学意义(P>0.05)。与正常对照组比较,H组、O组及HAO组baPWV升高分别为(1354.8±91.6)、(1366.1±81.7)、(1413.8±67.7)比(1206.2±168.0)cm/s,均P<0.01,HAO组又明显高于其他3组(均P<0.05)。析因分析显示,高血压和肥胖对网膜素1存在交互作用,肥胖无论是在正常血压还是高血压时均对网膜素1起单独效应,而高血压仅在非肥胖时对网膜素1起单独效应,在肥胖时则无单独效应。Pearson相关分析显示,网膜素1与BMI、收缩压、脉压、三酰甘油、总胆固醇、低密度脂蛋白胆固醇(LDL-C)、baPWV呈负相关,与高密度脂蛋白胆固醇(HDL-C)呈正相关(均P<0.05)。多元线性回归分析显示,BMI、收缩压、LDL-C、HDL-C是影响baPWV的主要因素(均P<0.05)。结论网膜素1水平下降与高血压及肥胖存在一定内在关系。但相较于高血压,网膜素1水平变化与肥胖的关系更为紧密。Abstract: Objective To assess the relationship between serum omentin-1levels and arterial elasticity among essential hypertensive patients with obesity. Methods A total of 255 subjects with normal blood pressure or essential hypertension were recruited,and they were divided into 4groups:the normal control(NC)(n=65),hypertension(H)(n=66),obesity(O)(n=63),and the hypertension with obesity(HAO)groups(n=61)according to blood pressure and body mass index(BMI). Serum omentin-1 was measured by enzyme-linked immunosorbent assay(ELISA)method and brachial-ankle pulse wave velocity(baPWV)by Colin Arteriosclerosis Instrument for all study subjects. Results Serum omentin-1levels were significantly higher in the NC group(233.7±57.5)μg/Lthan in the H group(176.7±53.4)μg/L,the O group(151.4±48.2)μg/Land the HAO group(143.3±47.2)μg/L,respectively(all P<0.01). Serum omentin-1levels were lower in the HAO group than in the H group(P<0.01)and exhibited a decreasing trend relative to the O group without statistically significant differences(P>0.05).Of four groups examined,brachial-ankle pulse wave velocity(baPWV)was highest in the HAO group (1413.8±67.7)cm/s,all P<0.05and it was higher in the H (1354.8±91.6)cm/sand O (1366.1±81.7)cm/sgroups than in the NC group(1206.2±168.0)cm/s(both P<0.01). Obesity was observed to affect serum omentin-1levels irrespective of the presence of hypertension,while hypertension played a contributing role only in non-obese subjects. Pearson correlation analyses showed that serum omentin-1was negatively related with BMI,systolic blood pressure(SBP),pulse pressure,triglyceride,total cholesterol,low density lipoprotein cholesterol(LDL-C),and baPWV,while positively related with high density lipoprotein cholesterol(HDL-C)(all P<0.05). Factorial analyses indicated an interaction between hypertension and obesity on serum omentin-1levels. In multivariate linear regression analyses,BMI,SBP,LDL-C and HDL-C were significantly associated with baPWV(all P <0.05).Conclusion Serum omentin-1may exert a negative effect on arterial elasticity hypofunction,as well as arterial elasticity. Moreover,the effect of obesity on serum omentin-1was more obvious than that of hypertension.
