两种血浆肾素、醛固酮检测方法对原发性醛固酮增多症筛查效率的评价
Comparison of plasma renin and aldosterone measured by radioimmunoassay and chemiluminescence immunoassay in screening primary aldosteronism

摘要: 目的探讨放射免疫法(RIA)与化学发光免疫法(CLIA)测定肾素、醛固酮结果的相关性以及对原发性醛固酮增多症(原醛)的筛查价值。方法对高血压患者96例分别采用RIA和CLIA平行检测血浆肾素活性或肾素浓度、醛固酮,对这两种检测方法的结果进行相关性分析,采用受试者工作特征(ROC)曲线比较这两种方法测得的醛固酮与肾素的比值(ARR)在原醛筛查中的灵敏度、特异度。结果 96例患者中41例诊断为原醛,两种方法测得的肾素活性(浓度)呈正相关(r=0.905,P<0.01),醛固酮浓度亦呈正相关(r=0.967,P<0.01)。两种方法计算ARR的ROC曲线下面积分别为0.94(95%CI0.89~0.99)和0.92(95%CI0.85~0.98),两者比较,差异无统计学意义(P=0.52)。筛查原醛RIA法ARR以30(nmol/L)/ng/(L·h)为切点,灵敏度为90.2%,特异度为92.7%;CLIA法ARR以25为切点,灵敏度为95.1%,特异度为83.6%。两种检测方法并联灵敏度为97.6%,特异度为83.6%。结论两种方法检测肾素、醛固酮具有良好相关性;筛查原醛,CLIA具有更高的灵敏度,但其特异度比RIA低。Abstract: Objective To examine the detection efficiency of radioimmunoassay(RIA)and chemiluminescence immunoassay(CLIA)methods in measuring plasma renin and aldosterone for screening primary aldosteronism(PA).Methods Plasma renin and aldosterone were measured in 96 patients with hypertension by RIA and CLIA methods,respectively. The correlation between the two methods was analyzed. The area under curve(AUC),as well as the sensitivity and specificity of the two methods were computed by receiver operating characteristic curve(ROC).Results Forty-one patients were diagnosed with PA. The correlation between the plasma renin and aldosterone concentration detected by two methods was positively correlated(r=0.905 and 0.967,respectively,both P<0.01).The AUC of ROC for the aldosterone to renin ratio(ARR)determined by RIA and CLIA methods screening PA were 0.94(95% confidence interval or CI 0.89-0.99)and 0.92(95% CI0.85-0.98),respectively,with no significant differences(P>0.05). The sensitivity and specificity were 90.2% and 92.7%for RIA method at the cutoff point of 30(nmol/L)/ng/(L·h)for ARR,and 95.1%and 83.6%for CLIA method at the cut-off point of 25 for ARR. The sensitivity and specificity of the parallel test were 97.6% and 83.6%. Conclusion Both RIA and CLIA methods are closely correlated in measuring plasma renin and aldosterone.Compared with RIA method,CLIA method has a relatively higher sensitivity but lower specificity in screening PA.
