Abstract:
Objective To explore the association between 1hpostprandial hyperglycemia during oral glucose tolerance test(OGTT)and left ventricular hypertrophy(LVH)in elderly patients with essential hypertension(EH).Methods A total of 562 elderly patients with EH were enrolled. All patients underwent OGTT for the evaluation of glucose tolerance. According to the OGTT results,patients were divided into normal glucose tolerance group(NGT,n=367),impaired glucose regulation group(IGR,n=127)and type 2diabetes group(DM,n=68).Based on the 1-h postload plasma glucose(1hPG)cutoff point of 11.1mmol/L,NGT subjects were divided into two groups:NGT-1hPG<11.1mmol/L(n=255)and NGT-1hPG≥11.1mmol/L(n=112)group. The indicators related to left ventricular structure were measured by echocardiography. Pearson correlation and multiple linear regression analysis were performed to assess the influencing factors of LVH. Results The left ventricular mass index(LVMI)and LVH prevalence in the NGT-1hPG≥11.1 mmol/L group were not different from those in IGR group or DM group (125.62±28.45)vs(125.89±28.31),(128.37±25.57)g/m2;38.39% vs 39.37%,42.65%,all P>0.05,but were significantly higher than those in NGT-1hPG<11.1mmol/L group(125.62±28.45)vs(112.54±26.18)g/m2;38.39% vs 24.71%,both P<0.05. Pearson correlation analysis showed that OGTT1 hPG was positively correlated with LVMI in NGT-1hPG<11.1 mmol/L,NGT-1hPG≥11.1 mmol/L,IGR and DM group(r=0.301,0.576,0.424,0.514,respectively,all P<0.01). Multiple linear regression analysis indicated that OGTT 1hPG was independently correlated with LVMI in NGT-1hPG≥11.1mmol/L,IGR and DM groups(B=1.268,1.250,1.316,respectively,all P<0.01). Conclusion OGTT 1hPG is closely correlated with LVH in elderly patients with EH. Enough attention should be paid to those subjects with increased OGTT 1hPG.
