分级诊疗体系下高质量血压管理1例

High-quality blood pressure management under a tiered healthcare system: a case report

  • 摘要: 在中青年高血压控制率低、常伴靶器官损害的背景下,本报告展示一例在分级诊疗体系下实现高质量血压管理的病例。患者为39岁男性,高血压3级伴肾功能不全(G2A3期)及大量蛋白尿。通过社区初筛、上级医院明确诊断为高血压性肾损害并启动以肾保护为核心的强化联合治疗方案,随后转回社区进行长期随访。管理过程中,依托智能技术进行密集血压监测以指导治疗。经过10周干预,患者血压、血压变异性及血压目标范围内时间均显著改善,蛋白尿大幅减少,肾功能保持稳定,年度评估显示靶器官损害无进展。本案例表明,分级诊疗框架下整合的协作管理模式、个体化药物治疗及技术辅助监测,能有效实现中青年高血合并肾损害患者的高质量血压管理与靶器官保护。

     

    Abstract: Against the backdrop of suboptimal hypertension control and frequent target organ damage in young and middle-aged adults, this case report demonstrates high-quality blood pressure management within a tiered healthcare system. The patient was a 39-year-old male with grade 3 hypertension, renal insufficiency (stage G2A3), and massive proteinuria. A collaborative model was implemented, involving initial community screening, definitive diagnosis of hypertensive renal injury at a tertiary hospital with initiation of an intensive, renoprotective combination therapy, followed by long-term follow-up in the community. Intelligent technology facilitated intensive blood pressure monitoring to guide management. After 10 weeks, significant improvements were observed in blood pressure levels, blood pressure variability, and time in therapeutic range, along with a marked reduction in proteinuria and stable renal function. Annual assessment confirmed no progression of target organ damage. This case illustrates that an integrated, collaborative management model within a tiered system, combined with individualized pharmacotherapy and technology-aided monitoring, can effectively achieve high-quality blood pressure control and target organ protection in young and middle-aged hypertensive patients with renal injury.
