运动血压监测与管理中国专家共识

Chinese expert consensus on exercise blood pressure monitoring and management

  • 摘要: 规律的运动锻炼是健康的生活方式,也是预防和控制高血压重要的生活方式干预之一。然而,运动存在不容忽视的潜在风险。运动中异常的血压反应,不仅是心源性猝死等运动相关急性事件的独立危险因素,还与左心室肥厚、冠心病、心力衰竭、脑卒中等心脑血管疾病密切相关。如何在高血压以及高心血管风险人群中开展运动血压监测是目前亟需解决的问题。本共识系统回顾现有文献,并提出符合中国人群特征的运动血压监测与管理方案,对在医疗机构中开展运动血压监测的设备、方法以及关键操作等做出明确、可操作的推荐。期望文件有助于在有运动需求的一般人群中,尤其是高血压以及高心血管风险人群,推广运动血压监测,帮助早期识别运动相关风险,优化运动管理方案,保障运动健康获益,全面提高高血压防治和控制水平。

     

    Abstract: Regular exercise is healthy in lifestyle, and is one of the important lifestyle interventions for the treatment and prevention of hypertension. However, there are considerable potential risks associated with exercise. Abnormal blood pressure responses during exercise are not only an independent risk factor for exercise-related acute events, such as sudden cardiac death, but are also closely associated with cardiovascular and cerebrovascular diseases, such as left ventricular hypertrophy, coronary heart disease, heart failure, and stroke. How to implement exercise blood pressure monitoring in patients with hypertension and those at high cardiovascular risk remains an unaddressed issue. This consensus aims to systematically review the current literature and propose a strategy for exercise blood pressure monitoring and management in the Chinese population. It also provides actionable recommendations regarding the equipment, methodology, and key operational procedures for exercise blood pressure monitoring in the clinical setting. It is hoped that this document would help promote exercise blood pressure monitoring in the general population with exercise needs, especially in patients with hypertension and those at high cardiovascular risk, and eventually help for the early identification of exercise-related risks, the optimization of exercise management, and the prevention and control of hypertension.
