高血压患者脂蛋白相关磷脂酶A2与肱踝动脉脉搏波传导速度及踝臂血压指数的相关性

Correlation between lipoprotein-associated phospholipase A2 and brachial-ankle pulse-wave velocity and ankle-brachial index in patients with hypertension

  • 摘要: 目的探讨高血压患者血浆脂蛋白相关磷脂酶A2(LP-PLA2)与肱踝动脉脉搏波传导速度(baPWV)及踝臂血压指数(ABI)的相关性。方法高血压患者144例作为高血压组(其中低危20例,中危30例,高危32例,很高危62例),30名门诊体检健康者作为对照组。所有研究对象行血脂、空腹血糖、餐后2h血糖等生化指标检查,应用酶联免疫吸附试验(ELISA)法测定LP-PLA2,应用动脉硬化检测仪检测baPWV及ABI。比较对照组与高血压组及不同危险分层高血压组间LP-PLA2、baPWV及ABI的差异,分析LP-PLA2与baPWV及ABI的相关性。结果高血压组LP-PLA2、baPWV高于对照组(268.62±87.13)比(187.64±49.01)μg/L,(1878±362)比(1398±239)cm/s,P<0.01,高血压组ABI低于对照组(1.13±0.07比1.19±0.06,P<0.01)。高血压很高危组LP-PLA2、baPWV高于高危组、中危组及低危组(289.19±95.37)比(253.70±80.38)、(233.37±55.14)、(219.62±50.55)μg/L;(1997.05±380.54)比(1772.56±292.27)、(1738.33±224.45)、(1569.80±200.66)cm/s;均P<0.05。高血压很高危组ABI低于高危组、中危组、低危组(1.10±0.07比1.14±0.05、1.15±0.04、1.17±0.04,均P<0.05)。高血压患者LP-PLA2与baPWV呈正相关(r=0.857,P<0.01),与ABI呈负相关(r=-0.436,P<0.01)。结论高血压患者很高危组LP-PLA2、baPWV升高,ABI降低,LP-PLA2与baPWV呈正相关,与ABI呈负相关。

     

    Abstract: Objective To investigate the relationship between lipoprotein-associated phospholipase A2(LP-PLA2)and brachial-ankle pulse-wave velocity(baPWV)and ankle-brachial index(ABI)in patients with hypertension.Methods A total of 144 hypertensives were recruited as hypertension group,of which 20 patients were at low risk,30 at middle risk,32 at high risk and 62 at very high risk. And 30 normotensives served as control. The blood lipid,fasting blood glucose,2-hour postprandial blood glucose were examined. The level of LP-PLA2 was detected by enzyme-linked immunosorbent assay(ELISA)method,and baPWV and ABI were measured by arteriosclerosis detector. The levels of LP-PLA2,baPWV and ABI were compared among hypertensives with different risk stratification and control group. The correlation between LP-PLA2 and baPWV or ABI was analyzed. Results The levels of LP-PLA2 and baPWV in hypertension group were significantly higher than those in the control group(268.62±87.13)vs(187.64±49.01)μg/L,(1878±362)vs(1398±239)cm/s,P<0.01. While ABI in hypertension group was lower than that in control group(1.13±0.07 vs 1.19±0.06,P<0.01). The LP-PLA2 and baPWV in the very-high-risk group were higher,while ABI was lower than those in the high-risk group,middle-risk group and the low-risk groupLP-PLA2:(289.19±95.37)vs(253.70±80.38),(233.37±55.14),(219.62±50.55)μg/L;baPWV:(1997.05±380.54)vs(1772.56±292.27),(1738.33±224.45),(1569.80±200.66)cm/s;ABI:1.10±0.07 vs 1.14±0.05,1.15±0.04,1.17±0.04;all P<0.05. The level of LP-PLA2 was positively correlated with baPWV(r=0.857,P<0.01)and negatively correlated with ABI(r=-0.436,P<0.01)in hypertensive patients. Conclusion LP-PLA2 and baPWV were increased and ABI was decreased in very-high-risk hypertensives. LP-PLA2 was correlated positively with baPWV,while correlated negatively with ABI.
